SELECT Statement of Informed Consent
The sponsoring organization has asked you to complete a survey using the SELECT System. SELECT is designed
to assist companies and other organizations in evaluating candidates.
While SELECT is a very useful tool, the results from this survey are only part of a complete evaluation.
Other factors - including your specific skills, experience, education, and job history - are not
evaluated by SELECT and will be considered along with your SELECT results to make a final evaluation.
The survey results belong to the sponsoring organization. You will not receive your survey results.
